Especially ethnic rhinoplasty operations require additional different techniques to be applied over the normal nasal aesthetic operations.

We performed revision ethnic rhinoplasty about 2 months ago and my patient came back again today to İstanbul. She is returning to England.

I would wish the patient to succeed. Greetings from Istanbul to my all patients and friends who live at England.

Two months after undergoing a revision rhinoplasty procedure, the patient's progress and healing can be assessed. At this stage, it is typical for some of the initial post-operative swelling and bruising to have subsided significantly. The nose is gradually taking on its final shape, although it may still be evolving as the healing process continues.

Patients should have a clearer idea of the results and improvements achieved through the revision rhinoplasty at this point. However, it's important to keep in mind that the full refinement of the nose may take several more months to become fully apparent. The extent of this gradual transformation depends on the specific techniques used during surgery, the individual's healing process, and the surgeon's skill.

During this two-month post-op period, it is essential for patients to continue following their surgeon's post-operative care instructions diligently. This typically includes maintaining proper hygiene, avoiding any activities or habits that could potentially damage the nose, and attending any scheduled follow-up appointments with the surgeon.

In terms of aesthetics, some residual swelling and minor asymmetries may still be present at this stage, but these issues are expected to improve over the coming months. Patients should remain patient and allow their bodies to heal naturally.

Two months after revision rhinoplasty, individuals can start enjoying the visible improvements in their appearance, but they should also remain aware that the final results will continue to refine and become more defined over time. Proper care and patience are crucial to achieving the best possible outcome from the surgery.

The term "ethnic thick-skinned nose aesthetic" refers to the physical characteristics and aesthetic preferences associated with noses that are commonly found in certain ethnic groups. It's important to approach this topic with sensitivity, as discussing physical features in the context of ethnicity can perpetuate stereotypes and biases. People's appearances are diverse and should be celebrated, regardless of their ethnic background.

That said, there is no single standard of beauty for noses, and what is considered attractive can vary greatly among individuals and cultures. Noses come in all shapes and sizes, and what is considered beautiful is highly subjective.

It's essential to promote body positivity and cultural diversity while avoiding harmful stereotypes or judgments about people's appearances based on their ethnicity. Beauty is subjective, and there is no one "correct" nose shape or size. Embracing and celebrating diversity is a more inclusive and respectful approach.

Differences of ethnic rhinoplasty with thick skin

There is no single standard technique for ethnic thick-skinned noses. The differences between thick-skinned nose aesthetics and standard rhinoplasty on thin-skinned noses can be listed as follows:

- Cartilage support is very important in noses with thick skin. The cartilage tissue under the nasal skin should be protected as much as possible. As a simple logic, the number and quantity of load-bearing steel columns in large buildings should be more than in other large and low-rise buildings, and when the cartilage support is weakened, collapse in the nose wings and sagging at the tip of the nose occur more easily. While thinning of the alar cartilages is sufficient to achieve thinning at the tip of the nose of a thin-skinned female patient; Weakening of these cartilage grafts in a patient with thick skin features may cause nasal valve collapse. In these patients, shaving of the fibroadipose tissue under the skin and additional cartilage graft applications at the tip of the nose may be needed.

- In patients with thick skin structure, "dead space" under the skin, that is, the emergence of a void area as a result of excision of a tissue during the surgical procedure or replacement of tissues in the subcutaneous area, may cause distress. In these patients, a round and fleshy nasal tip may appear as a result of the production of fibroadipose tissue inside the dead spaces during healing. In other words, in patients with thick nasal skin, "dead tissue" should be avoided as much as possible and cartilage graft applications may need to be performed more frequently than other rhinoplasty operations.

- It may be beneficial to use Isotretinoin Capsules (Roaccutane / Zoretanin) before and after surgery in patients with thick skin and acne-prone skin (https://www.ent-istanbul.com/2023/09/use-of-isotretinoin-capsules-roaccutane.html  You can take a look at the link).

- In patients with thick and oily skin structure, steroid injection can be planned 5-6 months after the surgery if they have a round and fleshy nose tip appearance (https://www.ent-istanbul.com/2019/05/steroid-injection-after-rhinoplasty.html - You can check out the link).

- In patients with thick skin, recommendations such as salt restriction, use of bromelian and quarcetin may need to be followed for longer than usual after surgery.

- It is necessary to explain to patients that the results obtained especially through rhinoplasty simulation or digital aesthetic applications may not match the actual surgery results. It may cause patients to have unrealistic expectations before surgery and disappointments after surgery.

- In patients with a large number of comedones at the tip of the nose when examined closely, it is almost impossible to obtain an angular nose tip due to the high camouflage effect of the skin after surgery in patients who are found to have a thick fleshy appearance under the skin and skin upon examination with the index finger and thumb.

- In patients with thick and oily skin structure, it is recommended to perform rhinoplasty with open technique and thin the nasal skin with bilevel skin dissection. Skin structure is genetic and during healing, the fibroadipose tissue removed by shaving can be regenerated by the body.

- After rhinoplasty operations with thick skin, external nasal pressure with bat application and cast can reduce dead spaces under the skin and reduce the occurrence of excessive nasal edema.
