Definition of The Nasal Septum Perforation
It consists of nose septum, cartilage and bone, and both sides are covered with mucopericondrium - cartilage gland, mucoperiosteum and it vertically dividing the nasal cavity along the length to two cavities.
Nasal septum perforation / perforation of the nasal cartilage is referred to as "nasal septum perforation / perforated nasal septum", which is the part of the nasal septum that divides the nasal cavity into two.
Nasal Septum Perforation Classification
Nasal septum perforations are classified into 3 categories according to their size:
- Small holes - Diameter less than or equal to 0.5 cm
- Middle holes - Diameter between 0.5 and 2 cm
- Large holes - perforations larger than 2 cm in diameter
The location and size of perforation affects the indication that may occur in the patient.
Nasal Septum Perforation Causes
Causes of nasal septal perforation can be classified into the following main categories:
Traumatic causes

Nasal Septum Perforation Symptoms
The punctures in the nose septum cause air leakage between the right and left nasal cavities. The air flow, which enters each nostril and is heated, refined, humidified, and transmitted back to the nasal passages, passes through the septum hole through the opposite side of the jet stream and proceeds without dewatering filtrationSmall holes in the front may make more noise.
Nasal Septum Perforation Treatment
Treatment is divided into 3 main categories:
1. Medical treatment
2. Insertion of septal button or septal prosthesis
3. Surgical treatment
Medical Treatment of Nasal Septum Perforation
Medical treatment is planned to reduce symptoms rather than correcting perforation. For this purpose, it is possible to use drops, sprays or ointments containing the mucosa moisturizing agent, vaseline, glycerin derivatives, a vitamin and e vitamin, which reduce the loss of fluid from the mucous membrane.
Septal Button or Septal Prosthesis for Nasal Septum Perforation
A septal button placement can be implemented as a temporary or long term solution. The soft silicone derivative buttons placed in the hole ensure that the hole between the nasal cavities is covered.
Nasal Septum Perforation Surgical Treatment - Surgery
Surgical repair can provide a definite solution, but the failure rate can be up to 60%, especially in large holes. The two main reasons for this high failure rate are inadequate blood flow and damaged surgery. More than 90% of small holes can be reliably closed.In modern 2-stage operations, success rates can exceed 80%.
Although many techniques have been described for repair of the perforation of the septum, no standard surgical protocol has been developed.
