We Performed the 4th Revision Rhinoplasty Operation
We Performed Micro-motor Assisted 4th Revision Nose Aesthetic Surgery With Rib Cartilage
The patient, who had undergone three rhinoplasty operations in different clinics before, was admitted because of asymmetrical nostrils and sagging of the tip of the nose. The following features were found in the examination of the patient:
- asymmetrical nostrils
- droopy nose tip
- features of thick and fibrotic skin
- wider than normal nasofrontal angle
- weaker than normal and soft nasal cartilage support
We planned the 4th rhinoplasty operation for the patient using micromotor, ultrasonic device and rib cartilages.
As seen in the video above and the photos below, the patient underwent the following procedures:
- bilateral internal low to high osteotomy
- bilateral external osteotomy
- reduction of the nasal root region by circling with the micromotor system
- shaving the roughness on the sides of both chin bones with micromotor system and thinning the nasal bone
- replacement and sewing of alar dome points
- right marginal rim graft placement
- columellar strut graft placement
- cap graft placement
- bilateral alar base resection
- After the procedure is completed, camouflage grafts are placed under the nose wings and fixed with a single 5/0 rapid vicryl from the nose skin.
After the operation was completed, bleeding was controlled, internal silicone splints were placed in the nose and splints were fixed to the anterior part of the nasal septum with 4/0 vicyrl.
The nose was bandaged using 3M Brown Micropore Tape and an external thermoplastic splint (cast) was placed.
In this patient, full recovery may take up to two and a half years due to thick skin features and repeated surgical trauma. Many care suggestions were explained to the patient, such as salt restriction in the diet, drinking green tea, eating pineapple, nasal massage with cold arnica gel, and nighttime sleep taping.

1st week control examination of the patient
The patient's external cast and nasal bandage were removed, a small amount of healing bruises under the eyes and swelling in the nose are seen.
