We Performed Revision Rhinoplasty Operation For Treatment of Drooping Columella + Alar Collapse

Revision Nose Aesthetic For Female Patient

The patient, who had undergone rhinoplasty in a different country 2 years ago, applied to our clinic because of nasal deformity and breathing problems. As a result of the examination of the patient, the following findings were found:

- drooping columella
- left nasal septal deviation
- weak nasal wing cartilages on the left, collapse observed while breathing (nasal valve collapse, alar collapse)
- asymmetry in the nostrils
- overprojected nose tip
- deviated nasal bone to the right

We planned an open technique secondary rhinoplasty operation for the patient. During the operation, The tongue-in-groove (TIG) techniquetechnique and septocolumellar suture technique were used to correct the drooping columella for upward movement of the columella.

Marginal rim grafts and alar batten graft were placed on the left alar cartilage outer crus. A bilateral thin spreader graft was placed. Bone protrusions on the nasal dorsum were removed with the micomotor system. For the tip of the nose to move backward and slightly downward, the alar cartilage dome points were changed and the overlapping technique was applied to the lateral crus.

As a perioperative finding, the patient's alar cartilages were highly asymmetrical. The outer leg of the left alar cartilage was thinner and higher in the horizontal plane. Alar base resection was not performed on the patient with the aim of obtaining a natural and functional nose.

You can view the patient's photos below. It is a difficult nose to achieve millimetric full symmetry.
