Natural Rhinoplasty For Asymmetric and Thick-Skinned Nose - Before and 8 Months After

I wanted to share with you an example of thick-skinned rhinoplasty with an asymmetrical structure.  I have attached pre- and post-operative photos of our patient who had open technique micromotor assisted rhinoplasty 8 months ago.  The patient's nasal tip angle, that is, the nasolabial angle, was already at a high level and we did not specifically raise the tip of the nose any higher.  Even after 8 months, the tip of the nose still looks edematous and it takes a while to complete the healing.  Since we do it with the help of a micromotor, roughness is not seen in the light reflection on the nose.  In the healing process, the healing characteristics of the patient are very important, especially in patients with exaggerated healing tissue production, sometimes asymmetrical protrusions on the nasal arch can occur.  The asymmetry between the nostrils of the patient was also quite high and it can be seen in the postoperative photographs that the asymmetry has almost completely disappeared.  The patient used Nostril Retainer for approximately 3 months, at least 8 hours a day in the first weeks, and then less.

The patient's nose photos before and after 8 months are shown below.  When the nose is disproportionate, it normally appears as the first area of ​​attention when you look at a person's face.  In patients with near-normal facial proportions, eyes, lips and teeth are usually noticed first;  the nose draws attention first in those with asymmetrical and out of proportion nose structure.  When you look at the photos of the patient, it can be noticed that after the nose surgery, the patient's face appears more symmetrical after the removal of the asymmetry in the nose, the eyebrows and eyes are seen more beautiful and more striking, and the light reflection from the tip of the nose while smiling makes the patient look like a more sympathetic personality.  Due to the asymmetrical and drooping nasal tip appearance before the surgery, patients may appear more nervous.

Natural rhinoplasty

Nasal aesthetic operations that are natural or close to nature or compatible with the face are rhinoplasty operations that are specially planned and planned anatomical changes are kept more limited than other nasal aesthetic operations. Here, the procedures are planned by considering the patient's facial proportions. An effort was made to obtain a nose shape that was compatible with the facial structure of the above patient, not overly curved and raised, and not small. Without changing the nasal tip angle of the patient much, the nasal arch was reduced, osteotomies, preader graft applications and other techniques were used to change the nasal axis.

Before and after visual reviews

Looking at the images above and below, it can be noticed that the following positive changes have occurred with the rhinoplasty operation:

- in frontal photographs, the light reflection, which appears more asymmetric downward on the nose, comes to the midline, making the nose appear narrower and shorter

- In diagonal images, the nasal hump is naturally reduced, and the drooping appearance of the nose is eliminated due to the nasal arch.

- When the photos taken from the top are compared, it can be seen that the nostrils are asymmetrical before the operation, the right nostril is rounder and larger, and in the post-operative images, the nostrils are almost close to each other.

- In both side photos, it can be noticed that the nose looks straighter thanks to the reduction of the nasal bridge without changing the angle of the tip of the nose, and there is no sagging at the tip of the nose when laughing.
