Open Technique Micromotor Assisted Female Crooked Nose Aesthetic

We Performed Rhinoplasty for a Crooked and Flattened Nose

We performed a micromotor-assisted rhinoplasty on a female patient who presented with severe nasal congestion, in addition to significant nasal deviation and a flattened appearance. Preoperative examination revealed the following:

- C-shaped asymmetric light reflection along the nasal axis (crooked nose deformity)

- S-shaped nasal septal deviation

- Weak tip support and a flattened (under-projected) nasal tip

- Caudal septal deviation (anterior septal dislocation shifting into the right nostril)

- Thick nasal skin

- Slightly prominent cheekbones and slanted eye shape (Asian facial features)

- Appearance of a relative nasal hump due to a drooping nasal tip

The patient underwent an open-technique, micromotor-assisted septorhinoplasty.

During the operation, the following procedures and graft applications were performed:

- Two-level subcutaneous dissection and subcutaneous tissue removal (to thin the skin)

- Partial hump reduction using a micromotor and bilateral osteotomy assisted by an ultrasonic device

- Additional external osteotomy

- Placement of a left spreader graft

- Repositioning of alar dome points

- Caudal septoplasty

- Placement of a septal extension graft

- Placement of a cap graft

- Placement of a left marginal rim graft

- Left alar base resection

A comparison of the patient's preoperative and postoperative images shows:

- The nasal axis has been aligned to the midline

- The nostrils appear much closer together

- Nasal projection has increased

Following the operation, the patient's nose was bandaged, and a thermoplastic splint was applied.
