After the incision which usually made behind the ear auricle, , the skin of the ear canal elevating to the front and the mastoidectomy surgery started with using of the tour as mastoid bone carving. Taking the back side of the external auditory canal and removing of middle ear structures depends on extension of middle ear infection. Eardrum and middle ear ossicles operations can be also performed while mastoidectomy operation.

- Mastoid bone disease (acute or chronic infection of the mastoid).
- Cholesteatoma
- A cochlear implant (bionic ear - inner ear prosthesis) in order to placing
- Tumors of the middle ear or nerve tumors (acoustic neuromas), meningiomas of the temporal bone paraganglioma (glomus tumors), removal of the lateral skull base tumors
- To repair the facial nerve
- Cerebrospinal fluid fistula (leakage of fluid surrounding the brain), then in order to repair

Cholesteatoma-related risks (Cholesteatoma is one of the most common indication for mastoidectomy)
In patients with middle ear cholesteatoma, cholesteatoma gradually grows over time and can cause serious complications such as:• brain abscess
• deafness
• vertigo (dizziness)
• damage to the facial nerve (causing facial paralysis)

Mastoid surgery should not be done in the following cases:
- In the presence of medical conditions that cause prevent general anesthesia
- Diseased only hearing ear
- Poorly pneumatized mastoid (less weathered) to be sclerotic (may be more difficult to determine certain anatomical points (otic capsule, facial nerve) in patients becomes more complex surgery.
- Sigmoid sinus (just behind the ear, a large veins)'s settlement
- Low roof of the middle ear (tegmen tympani) make it difficult to have surgery

Risks of Mastoidectomy Operations
Risks associated with mastoidectomy operations:• facial paralysis or weakness (a rare complication due to facial nerve injury)

• vertigo (dizziness; may persist for several days)
• taste change (food sour, metallic taste, or decreased sense of taste, this usually improves after several months)
• Tinnitus (abnormal noises in the ear, such as ringing, bursting and hissing)
The risk of mastoidectomy is less than the risk of cholesteatoma and chronic otitis media. Undoubtedly, the experience of the otolaryngologist with ear surgery, knowledge of anatomy and good hospital conditions will minimize these risks.

After surgery , the patient should be followed up according to follows:
Once a week (for first 3 weeks) patient must be come to doctor office.
Especially in the first 5 days after surgery, the following should be noted:
- Do not lift anything heavy.
- Avoid the sudden body movements if middle ear ossicles prosthesis be placed.
- Avoid sneeze with your mouth closed if eardrum surgery be performed.
- Protect your ear from water
- In the first week, the incision behind the ear, antibacterial ointment can be applied on sutures.
After surgery is normal to see the following:
- The first 10 to 14 days is normal ear discharge mixed with blood . Within days of discharge will be reduced.
- Equilibrium or dizziness may occur within a few days after surgery.
Nutrition :
There is no restriction of any diet. Solid, lint-free diet , constipation, may cause the patient's strain to ear drum movement. Soft , warm and high-fiber foods may be appropriate to take the first few days.
Drug Use:
Pain medication may be taken in case of pain other than aspirin . Generally, post-operative pain is not over . In particular the use of antibiotics to prevent infections after surgery is recommended.
After mastoidectomy surgery, the cavity may dry and complete recovery may take several months ...
After radical mastoidectomy and modified radical mastoidectomy operations, the newly formed bone wall cavity must be fully covered with epithelium and several months to complete the mucosal regeneration. During these few months, there may be occasional discharge of blood. It is not normal for an intense inflammatory discharge due to chronic otitis or cholesteatoma which can be seen before surgery.
