Important Role of The "Columellar Strut Graft" on Nasal Tip Elevation With Nasal Support

Nose Tip Lifting

Columellar strut graft is a cartilage graft which place between the medial cruras of alar cartilages. This graft can used for nasal tip support, treatment of nasal tip dropping, nasal tip elevation. Strut graft helps to control final nasal tip position.

I would prefer to use this graft almost in all my aesthetic surgery of the nose tip.

It providing a natural nasal support at the tip point. Some sources emphasized that, this graft can not increase nasal tip projection but it can very strongly support the nasal tip.

The bottom of this graft should be contacted with the maxillary crest and medial cruras of alar cartilages must be stiched symmetrically.

Nose Tip Lifting With "External Strut Graft"

This is one of the my favorite nasal tip lifting method. This procedure is simple, easy method and it can be performed together with septoplasty or any nasal surgeries.

Columellar strut grafts are placed between the alar cartilages medial cruras normally takes as a part of the in rhinoplasty procedure.

Skin on the nose tip is dissected and elevated to reveal alar cartilages.

It can provide a natural and symmetrical increasing to projection of nasal tip with increasing the nasal support.

How External Strut Graft Is Placed?

External Strut Graft

External strut graft can be placed in under local anesthesia with office conditions. After thelocal anesthetic infiltration on the nasal septum, cartilage graft which about 25 mm x 4 mm in size is taken. After a local anesthetic infiltration on columella approximately 3 mm a vertical skin incision is made on midpoint of columella. Tunnel opens upwards and downwards with the help of scissor. Cartilage graft is placed. Skin is sutured and the operation is terminated.

How Does The External Strut Graft Change The Nasal Tip Appearance?

External Strut Graft

External strut graft is placed forwarder than columellar strut graft. It can fill the nasal tip and it can provide a natural, symmetrical, permanent rise in nasal tip.

As seen in the photo above, tip of the nose is lifted without excessive tissue dissection.
External strut graft is placed after the deviated septum surgery in patient who is seen in the photo above . Please note that there not any swelling after this process and no the dark circles under the eyes of patients.

Advantages of the external strut graft technique:

- This procedure can be performed under local anesthesia and it usually done in an office setting
- External strut graft can cause the increase in nasal tip projection permanently
- It is a minimally invasive, Simple and easy tipplasty technique
- It does not require bandages or splints
- Swelling of the face and eye ecchymosis never occured after the procedure
- It can be combined with other nasal surgeries
- There is not any synthetic materials used

Nasal Tip Elevation With "Suspension Stitches"

There are some suture tip plasty technique exist. Some of them can be used only increse the nasal tip projection without any shape change at nasal tip.

Some of these suture tip projeciton techniques:

- Percutaneous suspension suture technique
- Securing nasal tip rotation through suspension suture technique
- Alar dome suspension stitches and caudal septal kartilage resection with suspension sutures

Additional modified suture tip increasing techniques can be defined but I don't like these techniques because of:

- I don't believe that these techniques can permanently increase the nasal tip
- Some permanent sutures can lead to tissue reactions
- Long-term results are unpredictable

Postoperative Patient Care After Nasal Tip Plasty

If nasal tip plasty operation performed with rhinoplasty operation, "instructions after the rhinoplasty operation" carefully should be applied.

It is normal to see for the following the nasal tip-plasty:

- Light to moderate bleeding from nose
- Swelling on nasal tip areas. The swelling on nasal tip may become worse 2-3 days after the operation, and it will start to decrease after the 4 days. Standing up (walking, sitting, etc) and head elevation at bedtime can help to alleviate the swelling.

Needs to be done after surgery:

- Ocean water sprays, antibiotics, pain medications and ointments containing antibiotics are generally recommended for use.
- If splint and tapes used, they will be removed by doctor generally 7 days after the operation.
- Patients can eat normal diet after the surgery. However, alcohol do not consumed for 1 month after surgery.
- Strenuous exercises, lifting heavy objects and bending head down can cause increase blood pressure on face and should be harmful for at least 1 week.
- Surgical area must stay dry for at least 1 week.
