How To The Septal Button Be Placed?
The local anesthetic is sprayed into the nose. Measure the upper and lower limit of the septum with a septum defect by placing a piece of paper in the nostril. The size of the holes can be marked using the marking pen from the other nostril. The flanges of the button are cut so as to be equal to the perforation and between the upper and lower limits of the septum. Smooth the edges of the button. The button is placed.
All tissue edges of the hole shell should cover enough to minimize crusting and bleeding. If the flanges remain too large, they may touch the region where the lower side cartilages are placed in the septum. This may cause pain and discomfort.
Who is Not Suitable for Septal Button Application (Septal Button Contraindications)?
The Septal button application is not suitable for patients with the following features:
• Patients with septum deviation are relatively contraindicated for button placement because they may cause poor seizures to the septum.
• Patients who are planning to septal perforation repairing surgery
• Patients with active infection in the nasal cavity
• Patients who use intranasal drugs actively
• Patients with active hemorrhagic septum. In these patients, autoimmune, infectious and malignant processes should be investigated.

Care Recommendations After Septal Button Application
What to do about patient care after septal button application can be summarized as follows:
• The patient should clean the nasal secretions by applying intranasal saline at least 3 times a day.

• Regular steam inhalations are recommended for 2 weeks (I have written that some clinics suggest overseas).
When Should The Septal Button Be Taken Off?
After the placing of silicon septal buttons, there are no strict rules about keeping a single septal button in place. This depends on the patient's ability to perform regular nasal hygiene. Some authors argue that the button will be removed and changed at regular intervals, while others will be able to stay in place for more than a year.
Risks of Septal Button Application
The risks associated with septal buttons can be summarized as follows:
• A few cases have been reported that displacement of the prosthesis towards the nasopharynx.
• Other long-term complications include increased nasal bleeding, increased crusting, intranasal pain, foreign body sensation, and increased wear of the perforation margins, extending the septum hole.
In the treatment of nasal septum perforation, if possible, prefer surgical repair. Nasal septal button application is more suitable in large holes and in cases where surgery is not appropriate.
