The pillar procedure is a small surgical procedure for the treatment of mild to moderate obstructive sleep apnea and to alleviate conventional snoring. Both snoring and obstructive sleep apnea may be caused by relaxation of the muscles behind your throat. The Pillar procedure involves the surgical placement of small polyester rods on the soft palate (a procedure involving the placement of prostheses such as injecting needle into the soft palate with sterile guns). Each implant is 18 millimeters (mm) in length - slightly less than an inch - and has a diameter of 1.5 mm. After the healing of the tissue around the implants, the soft palate hardens, reducing tissue relaxation and vibration. Pillar procedure is usually performed under local anesthesia and under office conditions. It can also be defined as soft palate prosthesis for snoring prosthesis or snoring.

Who Is Suitable For Pillar Implant Procedure?

If you have mild or moderate sleep apnea and you do not want to undergo surgery, a less invasive, non-surgical treatment such as continuous positive airway pressure (CPAP) may be attempted first. To prevent air clogging for the CPAP machine, your face mask is used to provide enough air pressure to your airline. Some people may need to use CPAP even after a pillar procedure is performed. If you do not want surgical procedure or CPAP and soft palate sagging is available, Pillar Implant Application can also be performed. Some physicians are planning to try this procedure if they cannot tolerate CPAP or Oral Device treatment.

Who Is Not Suitable For Pillar Implant Procedure?

Pillar Implant is not recommended for people with:

• People with severe obstructive sleep apnea
• Overweight or obese
This procedure can only be performed in patients with mild to moderate sleep apnea or simple snoring. Patients with severe sleep apnea or central apnea should not consider this treatment.

While sleeping, your muscles relax in the back of your mouth (soft palate), in the tongue and in your throat while relaxing. During snoring, when these tissues are weak enough, they vibrate and cause snoring. Snoring can cause distress to you and the person lying next to you. Snoring may also be an indicator of obstructive sleep apnea, a more serious disorder. If you have obstructive sleep apnea, open muscles in your mouth, tongue and throat do not contract enough or relax, the tissues in the airway are closed and clog your respiratory tract and leave you sleepless. This way, you may be exposed to 1 or more breathless breathing apnea attacks, meaning serious stress for your body. This may cause a sudden drop in the amount of oxygen in your blood. Sleep apnea syndrome is associated with serious health problems such as high blood pressure and heart disease.

Other symptoms of obstructive sleep apnea include restless sleep, frequent awakening, morning headache, drowning without stopping at nighttime breathing or stopping breathing and a feeling of daytime sleepiness.

If snoring is present or there are signs or symptoms of obstructive sleep apnea, consult with your doctor. You should probably consult with a sleep specialist and an otolaryngologist.

Pillar Implant Procedure, especially designed for patients with soft palate sagging, is a practical application that can be done in office conditions. In the soft palate tissue that vibrates and loosens, wound healing tissue is formed and this area is hardened. The effect of Pillar Implant Application is limited in patients with sleep apnea who have problems in different areas such as big tongue base and tonsil size.

How is Pillar Implant Applied?

What to do during the Pillar Procedure can be summarized as follows:

The Pillar procedure is performed in the surgeon's office, where you will be leaning back, after the teeth are brushed. During the operation, the surgeon takes less than 30 minutes:

• He applies topical anesthesia on the soft palate to numb the injection site.
• injects local anesthetic drugs into the area;
• Determine the position of the first implant
• First implant with single disposable insertion device
• Repeat the last two digits two more times

The American Food and Drug Administration (FDA) has confirmed that up to 5 implant applications can be performed at one time during the Pilar Iamplant Application. However, 3 implant applications are usually performed in one go. The number of implants added depends on the palate size and the physician's preference.

The fact is that these implants alone are not very effective in strengthening the soft palate. The procedure is actually based on your body reacting to attached foreign objects. Over time, hard tissues are formed around these implants and the palate hardens and the likelihood of loose dance in your respiratory tract is reduced.

Keep in mind that when your body starts to heal itself, the effects are not noticeable until 4-6 weeks after the procedure. It takes about 3-12 months for hard scar tissue to completely cover the implants.
When all is said and done, the implants should not be noticed or even visible (there is a possibility of spontaneous expulsion of implants coming out of the mucosa or end-to-end). Implants do not affect your ability to eat, drink or talk. At first you may notice a foreign body feeling behind your throat, but this feeling disappears.

Recommendations After Pillar Implant Application

Your doctor may ask you to sit for a few minutes to ensure that you do not experience significant bleeding or swelling. You can continue normal activities and eat normally the same day.
Your doctor can prescribe an anti-inflammatory painkiller drug to prevent swelling and can help any pain you feel after you have finished anesthesia. They may also ask you to make an antiseptic mouthwash for those who have had a pillar implant for several days and use an antibiotic to prevent infection.

Pillar Implant Risks

Studies have reported few complications with the pillar procedure. The most commonly reported complication is an implant tip (partial extrusion) that protrudes along the soft palate tissue. In this case, the implant can be removed or replaced.

Other potential complications are usually transient and resolved by themselves within a week or more after surgery. These:

• Sore throat
• Foreign body feeling in soft palate
• Difficulty in swallowing

As with any surgical procedure, infection is also a potential risk. If a fever develops after the procedure, you should consult your doctor. Pillar implant is an effective, reliable method in the long term; However, complaints such as dysphagia, foreign body sensation and dry mouth should be considered (detailed information is available on the following links).

As per your side effects, as you can imagine, you should experience sore throat after the procedure. In addition, an infection can often occur that is treated using antibiotics. In this case, there are cases where the body rejects the implants, in fact the implants move spontaneously and come out behind the throat. If this happens, they can be removed by your doctor when the tip of the implants is visible. You can find more information about the Pillar Implant Interpretations at the source links below.

Pillar Implant Application Can Be Combined With Other Snoring And Sleep Apnea Surgeries!

A variety of snoring and sleep apnea operations are available, including soft palate, small tongue, tonsils. Pillar implant application can be performed with these surgical procedures. The soft palate should not be shortened too much and should be left longer than the implant. There is no special requirement for hospitals or offices that perform pillar implantation. You can only perform the procedure under the clean office conditions.
