A Difficult Rhinoplasty Operation: 3rd Revision Rhinoplasty
Our patient, who had had nose rhinoplasty in two different countries before, was referred to us for treatment of current nasal distortion and breathing problem. As seen in the photo, there was asymmetry in the nose and a collapse in the right nose wing. We performed the third revision nose surgery using cadaveric cartilage. It was a difficult operation. In this case, the factors that I want to share with you and that cause the surgery to be more difficult than normal:
- the presence of insufficient amount of cartilage tissue
- two previous surgical trauma, anatomical changes
- tissue dissection is difficult due to suture reactions and healing tissue
- Serious asymmetrical areas, especially as seen at the tip of the nose
- collapse due to very small amount of cartilage tissue in the right nasal valve area
- thick skin at the tip of the nose
During the operation, the nose was reshaped by using rib cartilage, and the nose was stretched better in terms of aesthetics and functionality. Achieving complete symmetry in such surgeries is very difficult and unnecessary tissue dissections may cause wound complications. The stitches seen at the tip of the nose are self-melting. Edema appeared on the nose while photographing and video was taken.
