We performed an open technique septorhinoplasty operation for the deviated nose with thick skin features. The patient had severe nasal septum deviation and caudal septal dislocation due to a nasal trauma in childhood. During the procedure, nasal septum deviation treatment was also provided. Columellar strut graft application, cap graft application at the tip of the nose, alar cephalic resection and dome suture, alar base resection were performed in order to treat the flat and flattened appearance of the patient's nasal tip. The nasal wings were supported using cartilages obtained from the curved parts of the nasal septum in the nose. Bilateral bone cutting (oesteotomy) was made by opening a microtunnel through the nose.
