Rhinoplasty For Crooked Nose and Lip Nevus Removal
The patient, who developed nasal deformity and nasal obstruction due to previous nasal trauma, applied to our clinic for surgical treatment. Examination of the patient revealed caudal septal dislocation, severe septum deviation, and crooked nose deformity.
We performed an open technique septorhinoplasty operation on the patient. During the operation, it was found that the nasal septum was folded over itself and the nasal airway was quite narrow. Lesions compatible with nevus approximately 2 cm in diameter on the left side of the upper lip of the patient were removed during the operation with an optional biopsy and histopathological evaluation was reported as simple skin nevus.
As in the patient I shared with you here, it is not always possible to obtain a fully symmetrical result with technical operation in patients with severe asymmetrical and curved nasal deformity. In the post-operative photographs, it is seen that the nasal axis is completely centered and the light line of the nose is in the midline. However, after complete healing, there is a possibility that fewer than previous asymmetries may reappear. When you look at the photos before the operation, it can be easily noticed that there is a C shaped reflection in the nose (C shaped nose). This situation is usually due to nasal trauma that occurs in childhood, while nasal development continues. In some patients, cartilages in the nose may not be sufficient for reconstruction of the nose, even if primary rhinoplasty is present, and additional cartilage grafts may be required from the rib or auricle.
Since the patient has a fairly thick skin, cartilage grafts were prepared and placed on the nose wings, nasal tip and columella to support the cartilage support tissue as much as possible. It may not always be possible to obtain all these grafts from the nasal septum, which has a trapezoidal structure. Rib cartilage obtained from the patient's rib, the patient's auricle cartilage, or another human cadaver can be used. The thread seen at the tip of the nose is used to detect subcutaneous cartilage grafts, which are placed for camouflage.
At the tip of the nose, the dislocated caudal septum section was excised and removed, and a new "L strut graft" was formed.
