We performed an open technique rhinoplasty operation on the patient with very thick skin and an asymmetrical nose tip. We also performed bilevel underkin tissuse dissection and underskin tissue removal for the patient. In this patient, the underskin tissue taken from the nasal tip and supratip region was shifted to the upper side for nose root augmentation. You can see the stabilizing suture material at the root of the nose in the videos and photos below. We have used it as a natural and permanent filling material.
Although the skin structure is made thinner with this process; Since it is a genetic feature, there is a possibility that this tissue will be reproduced by the body, depending on the genetic skin structure characteristics inherited from the patient's mother or father. In order to slow down this wound healing and tissue production, there are many herbal and drug-produced tablet, gel and cream formations used in the internet and pharmacies. Most of the edema and bruise removal products used after surgery are products that slow down the work of fibroblast cells, which play a fundamental role in excessive collagen production and wound healing. There is no surgical procedure that can guarantee a permanently thin skin in patients with thick and oily nasal tip skin. However, using the open technique and bilevel skin dissection and underskin tissue resection may be beneficial. Again, in patients with thick skin, it is important not to leave a "dead space" under the skin. The goal here is to prevent the occurrence of abnormal healing arrow production that can fill these gaps. Again, in patients with thick skin features, as if a thick and solid column used when constructing a large and very heavy building; It is important to support the subcutaneous tissue by using thick and robust cartilage grafts. For this reason, while the nasal tip of the patient I shared with you was reshaped, different cartilage grafts were placed both on the outer crus of the alar cartilage and between the inner crus of the nose, on the tip of the nose. This is an operation in which very different techniques are used compared to the classical thin-skinned rhinoplasty. When you look at the photos of our patient's nose before the operation, you can see that the nostrils are in different sizes and one nostril is wider. We also performed a symmetrical alar base resection in order to both narrow the nostrils of the patient and make them as similar to each other as possible. We made more tissue resections from the nostril, which has a wide base, than from the inside. Another beneficial effect of alar base resection is the tightening effect on the outer part of the nasal wings, making the tip of the nose appear more triangular when viewed from the front. We performed a limited amount of hump reduction with a micromotor system for our patient, whom I share here. We prefer limited hump reduction, especially in patients of Asian origin. In case of overreducted hump, excessive flet and a flattened nasal corner may occur.

We aimed for our patient here to have a very natural ridge of the nose and typical male nose proportions. Below you can find the patient's pre-operative and immediately post-operative photos and videos, as well as the 1st, 2nd, 3rd and 6th month post-operative videos.
You can see that the patient's nose pain decreases over time. Note that in patients with very thick skin features, more than 2 years may be required for the nasal skin to fully heal and the edema to disappear completely. Generally, in these patients, although the difference between the images after 6 months and 2 years is evident; The rate of improvement and change in this period continues to be very slow.
