Fourth Revision Rhinoplasty With Rib Cartilage
We performed the 4th rhinoplasty operation on a patient who had previously undergone rhinoplasty operation 3 times in different clinics and had asymmetry in the nose and difference between the nostrils. Rib cartilage was used and only regional and local procedures were performed for minimal surgeon, trauma. Preoperatively, the patient has excess tissue in the supratip area, scar tissue in the left nostril, asymmetry in the nasal wings, and collapse in the right nasal wing. Cartilage graft applications for only these areas and bilateral osteotomy were applied to the patient. Micromotor-assisted smoothing of the bone tissue was performed. Considering the ethnic, thick-skinned nose features, ethnic wide facial structure and slanting eye structure, the nasal belt has not been reduced too much. On this page, you can find pre- and post-operative images and videos of the patient.
About the fourth revision rhinoplasty operation
Fourth revision rhinoplasty refers to a corrective nasal surgery performed after three previous rhinoplasty procedures. It is considered a complex and challenging procedure due to the alterations and scar tissue from previous surgeries.
The need for a fourth revision may arise if the desired outcome was not achieved in previous surgeries or if new issues or complications have developed. The goals of the fourth revision rhinoplasty are typically focused on addressing remaining aesthetic concerns, improving functionality, and enhancing overall nasal harmony.
During a fourth revision rhinoplasty, the surgeon will carefully evaluate the previous surgical outcomes, identify any residual asymmetry, irregularities, or functional issues, and develop a comprehensive surgical plan. Techniques such as cartilage grafting, tissue rearrangement, and precise adjustments to the nasal framework may be employed to achieve the desired outcome.
It is important to note that fourth revision rhinoplasty carries additional challenges and risks due to the altered anatomy, scar tissue, and compromised blood supply to the nasal tissues. As such, it is crucial to consult with an experienced and skilled surgeon who specializes in revision rhinoplasty to ensure the best possible outcome.
Prior to undergoing a fourth revision rhinoplasty, it is recommended to have realistic expectations, open communication with the surgeon, and a thorough understanding of the potential risks and limitations associated with the procedure. The surgeon will discuss the specific details, potential outcomes, and recovery process to help you make an informed decision.
Remember, the success of any revision rhinoplasty procedure depends on various factors, including the surgeon's expertise, patient cooperation, and realistic expectations. Open communication and collaboration between the patient and surgeon are vital to achieving satisfactory results in a fourth revision rhinoplasty.
Risks of fourth revision rhinoplasty
Fourth revision rhinoplasty carries additional risks and complexities due to altered anatomy, scar tissue, and compromised blood supply. Potential risks include poor wound healing, infection, asymmetry, breathing difficulties, and unsatisfactory aesthetic outcomes. Consulting with an experienced revision rhinoplasty specialist is crucial to minimize risks and achieve desired results.
Risks of recurrent revision rhinoplasty operations
Recurrent revision rhinoplasty, also known as multiple revision rhinoplasty, poses unique risks and challenges due to the complexity of the procedure and the presence of scar tissue from previous surgeries. Some potential risks associated with recurrent revision rhinoplasty include:
1. Poor wound healing: Scar tissue from previous surgeries can compromise blood supply and increase the risk of poor wound healing, leading to delayed recovery or wound-related complications.
2. Infection: With each revision procedure, the risk of infection may increase. Scar tissue can create a more favorable environment for bacterial growth, making infections more difficult to treat and increasing the risk of complications.
3. Altered nasal anatomy: Multiple revision surgeries can lead to further changes in the nasal anatomy. The risk of deformities, asymmetry, or functional issues, such as breathing difficulties, may be higher in recurrent revision rhinoplasty cases.
4. Scar tissue complications: Excessive scar tissue formation can create challenges during surgery and affect the final outcome. Scar tissue can make it more difficult to manipulate and reshape the nasal structures, potentially leading to suboptimal results.
5. Unsatisfactory aesthetic outcomes: Recurrent revision rhinoplasty cases may have a higher risk of not achieving the desired aesthetic goals. The complexity of the procedure and the presence of scar tissue can limit the surgeon's ability to achieve the desired changes, and the outcome may not fully meet the patient's expectations.
It is crucial to consult with a skilled and experienced revision rhinoplasty specialist who has expertise in dealing with complex cases. They can assess the individual situation, discuss potential risks, and develop a customized surgical plan to address the specific concerns and minimize complications. Open communication with the surgeon and managing realistic expectations are key to achieving the best possible outcomes in recurrent revision rhinoplasty cases.

Recommendations for patient care after fourth revision rhinoplasty
After undergoing a fourth revision rhinoplasty, it is crucial to prioritize proper care and follow your surgeon's recommendations for a smooth recovery and optimal results. While specific post-operative instructions may vary depending on your unique case, here are some general recommendations for patient care:
1. Follow your surgeon's instructions: Strictly adhere to all post-operative instructions provided by your surgeon. This includes guidelines for medication usage, wound care, activity restrictions, and follow-up appointments. Following their guidance is essential for a successful recovery.
2. Take prescribed medications: Take any prescribed medications as directed by your surgeon. This may include antibiotics to prevent infection, pain medication to manage discomfort, and any other prescribed medications for swelling or inflammation.
3. Manage swelling and bruising: Swelling and bruising are common after any rhinoplasty procedure. Apply cold compresses to the affected area as advised by your surgeon to help reduce swelling and alleviate discomfort. Avoid any activities that can increase swelling, such as bending over or strenuous exercise.
4. Protect your nose: Be cautious and take measures to protect your nose during the healing process. Avoid any activities or contact sports that could risk injury to your nose. Be gentle when washing your face and avoid blowing your nose forcefully.
5. Maintain proper hygiene: Follow your surgeon's instructions for wound care and hygiene. Keep the surgical area clean and dry to minimize the risk of infection. Use gentle, non-irritating cleansers and avoid applying pressure or trauma to the nose.
6. Eat a healthy diet: Maintain a balanced diet rich in nutrients to support your body's healing process. Stay hydrated by drinking plenty of water and avoid excessive salt intake, as it can contribute to swelling.
7. Rest and allow time for recovery: Take ample rest and avoid strenuous activities for the recommended duration. Allow your body time to heal and avoid any unnecessary stress on the surgical site.
8. Attend follow-up appointments: Keep all scheduled follow-up appointments with your surgeon. These visits are important for monitoring your healing progress, addressing any concerns, and making necessary adjustments if required.
It's crucial to communicate with your surgeon throughout the recovery process. If you experience any concerning symptoms or have questions about your recovery, don't hesitate to reach out to your surgeon for guidance and support.
In the fourth revision rhinoplasty operation, subcutaneous healing tissue and scar can complicate surgical dissection!
In a fourth revision rhinoplasty operation, the presence of subcutaneous healing tissue and scar tissue from previous surgeries can pose challenges during surgical dissection. These tissues can make the dissection process more complex and require careful handling by the surgeon.
The subcutaneous healing tissue and scar tissue can create adhesions and fibrous bands, making it difficult to separate and manipulate the nasal structures. This can affect the precision and control required to achieve the desired changes during the surgery.
The surgeon must navigate through the scar tissue carefully to avoid further damage or distortion of the nasal anatomy. They may need to use specialized techniques and instruments to dissect and separate the tissues, ensuring minimal disruption to the surrounding structures.
The presence of scar tissue can also affect blood supply to the nasal tissues, making it important for the surgeon to take precautions to preserve adequate blood flow during the procedure. Maintaining proper blood supply is crucial for optimal healing and reducing the risk of complications.
The surgeon's experience and expertise in revision rhinoplasty are particularly important in these cases. They must have a thorough understanding of the unique challenges presented by the scar tissue and subcutaneous healing tissue and be skilled in managing these complexities.
Preoperative planning and assessment play a vital role in addressing the challenges associated with subcutaneous healing tissue and scar tissue. The surgeon will evaluate the specific characteristics of the scar tissue and develop a tailored surgical approach to achieve the desired results while minimizing risks.
Open communication with the surgeon and a thorough understanding of the potential complexities involved are key to managing expectations and achieving the best possible outcome.
