In the photo above, we performed an upper lip lift operation in office conditions on the patient with a long and sagging upper lip structure, whose upper lip did not rise to the gums when smiling and appeared saggy. Before the procedure, the philtrum edges and lip arch lines were marked with a skin pen. After the local anesthetic injection, the skin and subcutaneous tissue were excised in the marked area. The procedure was completed after the suturing applied under the skin and to the skin. The upper lip area outside the suture was covered with steri strip tapes. The patient was advised to avoid water contact with the surgical field and to regularly cover the incision area with antibiotic ointments. The materials used in the skin incision suturation here may be absorbable or non-absorbable materials. When non-absorbable suture materials are used, it may be necessary to wait up to 14 days for the sutures to be removed.

Lip lift operations have gained popularity as a cosmetic procedure that enhances the appearance of the lips by shortening the distance between the upper lip and the nose, resulting in a more youthful and defined smile. Unlike lip fillers, a lip lift provides a permanent solution that doesn’t require frequent maintenance. This procedure, often performed in an office setting, offers numerous advantages for those looking to achieve a more attractive and balanced facial appearance.

What is a Lip Lift Operation?

A lip lift is a surgical procedure that reshapes the upper lip by removing a small strip of skin beneath the nose, creating a shorter philtrum (the area between the nose and upper lip). This results in a lifted appearance, exposing more of the upper lip's pink tissue, thereby creating a fuller and more youthful look.

The procedure is particularly beneficial for individuals with a long philtrum or those whose lips have thinned with age. Unlike temporary lip fillers, a lip lift offers a permanent enhancement, making it a preferred option for long-lasting results.

Advantages of Lip Lift in Office Conditions

Performing a lip lift in an office setting offers several advantages:

Convenience: Office-based lip lift procedures are typically quicker and more convenient than those performed in a hospital or surgical center. Patients can return home the same day, making it an attractive option for those with busy schedules.

Reduced Costs: Since the procedure doesn’t require a hospital stay, the overall cost is lower. Additionally, local anesthesia is usually sufficient, further reducing expenses.

Minimal Recovery Time: Recovery from a lip lift performed in an office setting is generally swift, allowing patients to resume normal activities within a few days.

Personalized Care: The intimate setting of an office allows for more personalized care, with direct communication between the patient and surgeon. This fosters a comfortable and reassuring environment.

Surgical Technique: How Lip Lift is Performed

The lip lift surgery typically involves the following steps:

In the photo above, there is the upper lip appearance of the patient who underwent surgical upper lip lift before and after the operation. It can be easily noticed that the upper lip distance is clearly shortened and the upper lip is directed upwards.

Consultation and Planning: Before the procedure, the surgeon evaluates the patient’s facial anatomy and discusses their desired outcome. This helps in planning the incision placement and amount of skin to be removed.

Anesthesia: The procedure is usually performed under local anesthesia, which numbs the area but allows the patient to remain awake and comfortable.

Incision: The most common technique is the "bullhorn" or subnasal lip lift, where a curved incision is made along the base of the nose. This method hides the scar in the natural crease where the nose meets the upper lip.

Skin Removal: A small strip of skin is removed from the area between the nose and the upper lip, depending on the desired amount of lift.

Lifting and Suturing: The upper lip is gently lifted, and the incision is closed with fine sutures. This results in a shorter distance between the nose and lip, and a more pronounced upper lip.

Final Adjustments: The surgeon makes any necessary adjustments to ensure symmetry and balance before completing the procedure.

Skin Suture Techniques

The success of a lip lift operation heavily depends on the surgeon's skill in suturing. Several techniques may be used to minimize scarring and ensure a smooth result:

Layered Closure: The incision is closed in layers, starting with deep sutures to support the lifted tissue, followed by superficial sutures to close the skin. This technique helps in distributing tension and minimizing scar formation.

Fine Sutures: Fine, non-absorbable sutures are often used to close the skin. These sutures are typically removed after a few days, leaving a minimal scar.

The scars due to comb-shaped skin sutures usually decrease considerably when the sutures are absorbed or removed.

Running Suture Technique: A continuous running suture may be used for the skin closure, which can reduce the risk of scar widening and promote faster healing.

Interrupted Sutures: Some surgeons prefer using interrupted sutures for more control over the final appearance of the incision. Each stitch is individually tied, which can allow for precise adjustments.

Skin Glue: In some cases, skin glue may be used alongside sutures to reinforce the closure and reduce scarring further.

Indications for Lip Lift Surgery

A lip lift may be indicated for individuals with the following concerns:

Long Philtrum: A long distance between the upper lip and nose can make the face appear aged or unbalanced. A lip lift shortens this distance, enhancing facial harmony.

Thinning Lips: As we age, the lips naturally lose volume. A lip lift can expose more of the lip’s pink tissue, creating the appearance of fuller lips without fillers.

Desire for Permanent Results: Unlike fillers, which require regular maintenance, a lip lift provides a permanent solution for lip enhancement.

Downturned Mouth Corners: A lip lift can also correct a downturned mouth by slightly elevating the corners of the lips, resulting in a more youthful and cheerful expression.

Previous Lip Filler Dissatisfaction: Patients who have had lip fillers and are dissatisfied with the results or want a more natural look may opt for a lip lift as an alternative. If you are planning a surgical lip lift, avoid unnecessary lip filler applications beforehand. Injecting excessive amounts of lip filler may affect the surgeon's vision during the lip lift operation.

Potential Complications of Lip Lift Surgery

While a lip lift is generally considered safe, like any surgical procedure, it carries some risks. Potential complications include:

Scarring: While efforts are made to hide the incision, scarring is possible. However, with proper care, scars typically fade over time.

Infection: As with any surgery, there is a risk of infection. This can be minimized by following post-operative care instructions and maintaining good hygiene.

Asymmetry: There is a risk that the lips may heal unevenly, leading to asymmetry. This can usually be corrected with minor touch-up procedures. Before the incision, marking the philtrum lines and the corners of the upper lip with a skin pencil and re-suturing the ends of these incisions can reduce the risk of asymmetry.

Numbness or Sensitivity: Temporary numbness or sensitivity in the upper lip area is common but usually resolves over time.

Overcorrection: In rare cases, the lip may be lifted too much, resulting in an unnatural appearance. A skilled surgeon will carefully plan the procedure to avoid this outcome.

Post-Surgical Care Recommendations

Proper post-surgical care is crucial for a successful recovery and optimal results. Here are some general recommendations:

Follow Surgeon’s Instructions: Adhere to all post-operative care instructions provided by your surgeon, including medication use and activity restrictions.

Keep the Area Clean: Gently clean the incision site as instructed to prevent infection. Avoid applying any harsh products to the area.

Apply Ice: Ice packs can help reduce swelling and discomfort during the first 24-48 hours after surgery.

Avoid Strenuous Activities: Refrain from any activities that may strain the lips or face, such as heavy lifting, vigorous exercise, or excessive talking or smiling, for at least a week.

Sleep with Head Elevated: Keeping your head elevated while sleeping can help minimize swelling.

Limit Sun Exposure: Protect the incision site from direct sunlight to avoid darkening of the scar. Use sunscreen or wear a hat when going outside.

Attend Follow-Up Appointments: Regular follow-up visits with your surgeon are essential to monitor healing and address any concerns promptly.
