Control hearing test (audiometry test) of a patient who had undergone endoscopic eardrum surgery to his right ear 2 months ago. When the eardrum was closed, the hearing was almost normal. Endoscopic ear surgery does not usually require an externally visible skin incision. The procedure is performed completely through the external ear canal. We also planned endoscopic membrane surgery in the other ear of our patient.

The reddish color of castellani solution, an antiseptic dye dripped into the patient's ear, can be seen in the cartilage graft placed at the site of the closed eardrum.

In endoscopic eardrum surgeries, there are advantages such as making the eardrum visible directly by the endovision prompt and not requiring additional skin incision except the incision in the cartilage graft area. Particularly in incisions made behind the auricle, there are disadvantages such as the need to press the auricle for several weeks, excessive pain and long recovery time. Endoscopic eardrum surgeries are suitable for selected patients. It is the most accurate that your physician recommends.

Treatment of eardrum with hole without incision and suture

The repair of the hole in the eardrum is generally done by 2 methods. The first is the operation performed with an incision made behind the auricle, also called the classical method. The second method is the repair process which is entered and performed with the endoscope, ie camera, through the ear hole without any incision.

First, the most important difference is that the incision has not been performed. As a result, the problem of suturing is eliminated. With this method, the duration of the operation and the time the patient receives anesthesia are severely reduced and the pain problem is very little because there is no incision. Why do you need ear surgery?

Surgery may vary depending on the situation. In other words, in cases where the problem is limited only to the eardrum, the protection of the ear from the water and if there is no inflammation, surgery can be performed at the request of the patient. Hearing loss is around 30% in simple membrane holes. This group is not necessarily included in the group that requires surgery. However, if there is inflammation that develops cholestatoma and melts the middle and inner ear bones, life-threatening conditions may occur and these patients should be operated. In addition to hearing loss, recurrent malodorous ear discharge and chronic infections are usually seen in the presence of cholestatoma. In this case, instead of simple eardrum repair, eardrum repair or mastoidectomy operations are performed alone.

Why is the eardrum perforated?

Perforation of the eardrum may occur due to trauma, ear diseases, confusion with foreign bodies, exposure to loud sounds. As a consequence, hearing loss and recurrent ear infections due to fluid escaping to the ear may cause serious problems for the patient.

In what cases should the eardrum repair be performed?

If there is a hole in the eardrum that is protected from water or there is no danger of infection in the nasal sinus area, if the ear discharge repeats itself, it may be necessary to close the hole in the membrane in order to improve the quality of life and prevent hearing loss. If the patient has significant hearing loss, problems in the ossicles conducting the sound can be eliminated with the same operation.

What is the appropriate age for eardrum surgery?

The ideal time for simple eardrum replacement surgery is recommended after 12-14 years of age, but patients with advanced inflammation should be operated at any age (sometimes under emergency conditions).

What should I pay attention to after surgery?

In general, it is necessary to avoid influenza infections in the first month. In addition, you should be protected from taking a blow to the ear for this purpose there is a double struggle and possible to take a blow as long as your doctor says you should stop the sport branches. You should not clog your ear canal with vaseline cotton until the doctor says otherwise. High and low pressure changes should be avoided for this purpose 4-6 months should be avoided for road trips and especially air travels that may change altitude.

Symptoms of acute eardrum rupture

The following complaints should be taken into consideration in order to understand that the eardrum is perforated:

Ear-pain, discharge, small or large-level hearing loss, frequent tinnitus, dizziness and consequently nausea and vomiting complaints, facial loss may occur, such as ear sounds previously heard in the ear doctor should be examined. The tympanic membrane treatment depends on the size of the tear, and if a small tear occurs, it may be closed spontaneously and may not require surgery. If the tear is large, different surgical procedures are performed.

Apart from surgical interventions, rupture can be closed within 1 week with antibiotic treatment, but inadequate treatment or ear leakage may cause chronic eardrum rupture. Following surgical procedures, the patient should be applied at home; The head should be supported with pillows while resting. Exercise movements and daily activities that increase ear pressure should be avoided.

The mouth should not be closed when sneezing is needed. The drug should be used for 2 weeks to prevent severe coughing. Surgical procedure should not be used for 2 months. The ear should be cleaned with the ear cleaning method recommended by the doctor.
