Revision Septal Pefroration Operation


Before and after the patient who has undergone several nasal surgeries in different clinics before and has undergone 2 unsuccessful nasal septum perforation closure procedures.

You can watch the endoscopic video made three and a half months after the open technique nasal septum perforation repair procedure applied to the patient, and you can see that the perforation is completely closed. The size of the perforation in the horizontal plane was around 2 cm.

The patient was recommended to irrigate the nose with a mixture of ocean water spray, rifampicin and isotonic serum before the procedure. After the procedure, silicone plinths in the nose were kept in the nose for approximately 3.5 months. During this time, the patient continued to irrigate the nose with antibiotic serum and ocean water sprays. It was learned that nasal septum perforation closure was performed using simple mucosal flap sliding techniques, which were previously performed in other clinics. During the open technique nasal septum perforation operation performed on the patient, the rib cartilage and underskin tissue obtained from the nasal tip skin were also used to close the hole in the nasal septum. Nasal septum perforation is a condition that causes adverse effects on nasal functions in varying amounts depending on the location and size. Especially in patients with nasal septum perforation located in the anterior and large dimensions, symptoms such as increased air flow velocity entering the nose and drying in the nose, feeling of air hunger, drying of the nasal passage may occur, and negative structural changes in the nasal mucosa may occur over time. Although it is usually a condition that can occur after nasal surgery, it can be seen after cocaine use, prolonged contact with chemical irritants, and the use of various nasal sprays.

As the size increases in nasal septum perforation, the chance of surgical success decreases. Especially when there is a large nasal septum perforation with previous surgeries, the chance of complete closure of the hole with millimetric success with a single operation decreases.

Closure of nasal sptum perforation can be done with open technique and closed technique (through the nose). Various cartilage graft applications and mucosal flap techniques can be used. Especially in patients with symptomatic septum perforation for a long time, bleeding, drying, crusting and structural changes may occur on the perforation edges over time. Especially in small perforations located in the front, whistling sound can be heard.
