Complicated Nose Surgery - Revision Nose Job For Crooked Nose + Septum Hole Closure


More difficult and complicated nose operation than normal. In the video and photos below, I have shared with you a patient who had a previous rhinoplasty operation and subsequently had a nasal deformity due to a traumatic nose fracture, and also perforation of the septum. The patient had a complaint of severe nasal congestion. There were functional problems in the nose due to nasal valve collapse on one side and nasal septum deviation on the other. When you look at the frontal photograph of the patient's nose, you can see the bright and shadowy areas at the same time. You can easily notice that there are serious asymmetries in the nasal bone, supratip cartilages and nasal type cartilages. We performed open technical revision rhinoplasty operation and nasal septum perforation closure operations for the patient. Rib cartilages were used. A credible example that is not easy to obtain a perfectly symmetrical result. There are collapses, collapses and protrusions in different areas on the nose.

The patient, whose photos I shared above, was recommended to keep the nasal splints placed in the nose longer than normal and to irrigate the nose with a mixture of rifampicin + isotonic saline solution. In this patient, the cause of nasal septum perforation was nasal surgery trauma, which had been previously performed in a different clinic.

Although the patient's nose is quite edematous in the photos above, I wanted to share it. The asymmetry between the holes at the tip of the nose of the patient seems to be quite resolved. It is not possible to make a definite comment due to edema, but you can examine the nose with a wide angle in the video below.

Before and 10 days after nasal septum perforation closure

The nasal septum perforation seen in the tomography section below was approximately 1x1.5 cm in size. During the operation, a cartilage graft was placed in the vertical plane and underskin tissue was placed in the suprtip region on both sides.

The video below was taken during the endoscopic examination immediately after the nasal silicone splints were removed (10 days after the operation). On both sides, a whitish raised area attached to the underskin tissue is seen. It is seen that the perforation is completely closed. The patient was advised to continue with intranasal care recommendations for several months to complete epithelization.


The patient was re-taped on the nose 10 days later, and it was recommended to use a nostril retainer. You can see the photos below.
