Nasal septum deviation as congenital or acquired due to trauma to the nose.
The symptoms of nasal septal deviations are summarized below (Source: Deviated Septum: Symptoms, Causes, and Surgical Repair - WebMD):

- The most common symptom of a deviated septum is nasal congestion, with one side of the nose being more congested than the other, along with difficulty breathing.
- Recurrent or repeated sinus infections

Other symptoms include frequent:

- Nosebleeds
- Facial pain
- Headache
- Postnasal drip
- Loud breathing and snoring during sleep
- Mild to severe loss of the ability to smell

A deviated septum may also cause sleep apnea, a serious condition in which a person stops breathing during sleep.

Nasal septum deviation, is a condition that affects quality of life.

Negative Effects of Nasal Congestion on Health

Nose is a very important organ that moisturizes, purifies and heats the air taken from the external environment and performs the scenting task. In people who cannot breathe nasal breathing due to nasal septum deviation and breathe through the mouth, it is inevitable that dirty, dry and germ air will reach the lungs and end airlines.

However, it has been shown that the air taken from the nose reaches up to the end airways in the lungs and is absorbed in a greater proportion of the oxygen. In patients with nasal obstruction, diseases such as mouth open sleep, deterioration in sleep quality, and chronic pharyngitis may occur.

Diagnosis of Deviated Nasal Septum

In patients with nasal septum deviation, a simple rhinoscopic examination or nasal endoscopic examination is usually sufficient. This is also important in assessing where the septum deviation is, the nasal valve area, and the turbinates. Paranasal sinus computed tomography can be performed especially when there are polyps or other obstructions in the nose, recurrent episodes of sinusitis or if endoscopic sinus surgery is also planned. Some private health insurances may require paranasal sinus tomography and radiology report to identify the preoperative disease. It is best to avoid unnecessary radiation exposure and it is not necessary to ask for a CT scan for every patient. In the video above, paranasal sinus ct images are available. The patient has nasal septum deviation and turbinate hypertrophy. The patient's intranasal air passage appears to be narrowed.

Computer Tomography Imaging Before The Septoplasty Operation

Paranasal sinus computer tomography imaging is necessary in surgical planning, for treatment of medical and surgical complications, and in all aspects of the complete management of adults patients with sinonasal and skull base pathology. Detailed information on the nasal septum and sinuses can be obtained (Source: Clinical consensus statement: appropriate use of computed tomography for paranasal sinus disease).

Above, it is seen that paranasal sinus screening coronal ct scans of patient with septal deviation to the right, the left middle turbinate concha bullosa anomaly and bilateral inferior turbinate hypertrophy.

Technique of Septoplasty Operation

Septoplasty is a corrective surgical procedure done to straighten the nasal septum. Operation can be performed under general or local anesthesia. ENT doctor works through the nostrils, making an incision in the lining of the septum to reach the cartilage targeted in the operation. Sufficient cartilage is preserved for structural support. After the septum is straightened, it may then be stabilized temporarily with small plastic tubes, splints, or sutures internally (Source: Septoplasty - Wikipedia, the free encyclopedia).

Caudal Septoplasty and External Strut Graft Technique
Caudal septum deviation is nasal septum devation at the bottom side of septum and which is a condition that can affect the nasal valve region with nasal tip support.
Sometimes open techniques require to correct this septum deviation. I generally prefer to closed techniques.

Is Nasal Septum Deviation Repeat After Septoplasty?

Normally, after the septoplasty operation for the surgical treatment of nasal septum deviation, the nasal septum that is left behind is not repeated without a trauma. A septum deviation may occur when a nasal septum may cause a change in shape after a septoplasty operation. "Turbinates" may vary depending on the contact with contaminated air and spontaneous growth due to allergic reactions. Septum deviation does not occur spontaneously!

Revision Septoplasty Operation

Revision Septoplasty Operation in Istanbul

Normally, after the septoplasty operation is performed, it is not easy for the septum cartilages to warp or bend spontaneously. When trauma or infection is added, septum deformation or dviation may occur. In the photo above, there are intranasal photographs of a patient who underwent septoplasty and turbinatectomy operations in another clinic under local anesthesia.

The photo on the right (right nasal cavity), yellow dashed lines show deviation of the nasal septum and the red arrow shows the excessively reduced right inferior concha, and the green arrow shows the adhesion (synechia) between the right lareal nasal wall and the septum.

Photograph on the raft (left nasal cavity), yellow dashed lines show nasal septum deviation. This nasal cavity has more than normal air cross-sectional area.

A revision septoplasty operation was planned for the patient.

Does The Nose Shape Change After Septoplasty Operation?

Normally, the nasal septum deviation is performed with closed technique and through the nasal cavity. The nose shape is not changed after the closed technique classical sepoplasty operation, since no question is made to the cartilage and bone which is the question of the nose. However, nose tip lifting may seen with the reconstruction of the nasal tip support cartilages after the open technique septoplasty operation.

When Can Internal Nasal Splints After Septoplasty Operation Be Removed?

After the septoplasty operation, it is usually implanted in the nasal silicone splints into the nose. These products have a round-shaped air duct that allows patients to breathe through the nose. Generally, these products are taken off within 5-10 days after septoplasty operation. In patients with advanced nasal septum deviation, it may be planned to keep them for longer and to keep them for a short period of time in simple septum deviations.

The Bruising Does Not Occur After Septoplasty Operation!

During septoplasty operation, no intervention is made to the nasal bone or cartilage of the nasal structure, and no bruise around the face or eye occurs after septoplasty operation.

Why Performing Septoplasty Operation in Children is Inappropriate?

Why Performing Septoplasty Operation in Children is Inappropriate?
One of the growth points in the nose is located at the anterior lower part of the nasal septum. There is a risk of damage to these growth points during deviation surgery in children, where the skull and bone structure grows. Some physicians argue that in case of limited cartilage resection, these surgeries can be performed in children; The general view is that these operations are not performed in the first 18 years of life. In patients with severe nasal septum deviation, there are some sources suggesting that a limited amount of cartilage removal should be performed. The general idea is to wait for the completion of the nasal development as much as possible.

Positive Health Effects of Septoplasty Surgery

Following septoplasty operation, the following positive effects on health arise due to the disappearance of nasal congestion:

- Improved sleep quality, reduced morning fatigue
- Decrease in complaints such as burning in the throat and drying
- Increased capabilities of walking distance and running distance
- Reduced frequency of lower respiratory tract infection
- Reduction in problems of chronic pharyngitis, tooth decay, such as tooth decay due to problems seen in drying

In patients with deviation surgery, an increase in quality of life and many permanent and positive health effects are determined.

Postoperative Care After Septoplasty Operation

There were no additional care recommendations after septoplasty operation.

If the operation was performed under general anesthesia on the day of surgery after the septoplasty operation, it should be open and dehydrated for the first 4 hours. On the day of surgery, general anesthesia-related nausea, mild dizziness and a small amount of headaches may occur. It is normal for bleeding to be seen from the front and back of the nose. Generally fhe first three days after the septoplasty operation, leakage or dripping bleeding is observed.

After nasal septoplasty operation, I usually prefer to put silicone splint into the nose. The two main reasons that I prefer this are that it does not cause pain when removed and prevents adhesions in the nose. Nasal silicone splints usually remain in the nose for 7 days. After septoplasty operation, silicone splints in both nasal cavity are removed from the private ent office of Dr.Murat Enoz.

The most important thing that needs to be done after septoplasty operation is the washing of the nose with saline water and the use of intranasal moisturizing drugs. Regular squeezing of the ocean water sprays, especially in the nose, prevents nasal crusting and increases the mucosal feeding

Drugs and their use after septoplasty operation:

- ocean water sprays 4 times a day in each nostril 2 times
- humidifying nasal drops (nazalnem drops) drops 4 times a day in each nostril 4 times a day
- a tablet containing paracetamol, 3 times a day, 1 tablet after the eating
- antibiotic tablets, 2 tablets per day, 1 tablet per day, after the eating

After surgery, you will be explained in detail with video recording, how to use these drugs.

How is the bath done after septoplasty operation?

After septoplasty operation, it is ideal to avoid city water, shampoo or soap contact within the nose for the first 3 weeks.

For the first 3 weeks you can tilt your head backwards, separate your hair and your body separately as in the hair salon. During the bath, it is appropriate that you do not use hot water.

Inappropriate food and drinks after septoplasty operation

Food and beverages that are inconvenient after septoplasty operation

Since it may increase the likelihood of bleeding, consuming all hot beverages and foods is inconvenient after septoplasty.

It is ideal not to be consumed in highly spicy, salty products. Again, it is appropriate to not be consumed fish oil, ginklo biloba, cherry and garlic, which may cause bleeding time.

What to consider when sneezing after septoplasty operation!

After septoplasty operation, internal nasal silicone splints placed inside the nasal cavity may cause sneezing attacks by contacting the bristles inside the nose or in patients with foreign body mechanism.

When the sneezing sensation comes after septoplasty operation, the mouth must be fully opened and the nostrils should not be closed. If the nostrils are closed during sneezing, a severe pressure increase in the nasal cavity and severe nosebleed bleeding may occur.

Simple recommendations to improve recovery rate after septoplasty operation

Increasing the mucosal moisture in the nose after septoplasty operation increases the rate of improvement. You can note the following recommendations:

- regularly use the recommended intranasal seawater sprays and nasal moisturizing products

- consume high fruits and vegetables, plenty of water (beneficial for the health of the mucous)

- avoid diuretic infusions and beverages (tea, coffee, instant coffee ...)

- It is ideal to avoid a climatic environment, an electric heater or a terrestrial cold climate that
accelerates the loss of moisture from the nasal mucosa. If you are going to be in such a way after septoplasty operation, you can use nasal moisturizer more often in consultation with me

- do not insert napkins, fingers or paper towels that dry the mucosa into the nose

About exercise and walking after septoplasty operation

After the septoplasty operation, you can walk regularly from the second day after the surgery. For this reason, it is appropriate to use nasal moisturizer products and drink water before walking.
After septoplasty operation, it is ideal to start lifting or heavy exercises after the 3rd week.
There is a risk of hemorrhage for the first 10 days after septoplasty operation. After 10 days the risk of nosebleeds decreases. Exercise and walking in cold conditions reduces the possibility of bleeding.

Flight after septoplasty operation

After septoplasty operation, the nasal silicone splints in the nose are removed and the nasal cavity is aspirated and cleaned. After the 1st week and after the cyclical splints have been removed, there is no harm in air travel. It is useful to use oil-based humidifier before air travel.

About Total Recovery Time After Septoplasty Operation

Factors affecting recovery time after septoplasty operation:

- compliance of the patient with the recommendations on intranasal care
- the healing characteristics of the patient
- humidity and other climate characteristics of the environment in which the patient lives
- healing features of the patient
- amount of surgical trauma

Depending on these characteristics, total recovery after septoplasty operation can be 6 weeks and longer. The recovery period may be prolonged, especially if you keep the nose dry and do not use moisturizing nasal drops. However, if you live in a house that is damp and close to the sea / ocean side (if you swim directly in the ocean or at sea), the total recovery time can be much shorter.
